	PHYSICALS: Individual physicals are REQUIRED to play any sport. Students without physicals will have to wait to participate in any sport until after a physical is complete. Physicals are available at Youens & Duchicela Clinic. Call 725-8545 for an appointment.

	IMPORTANT NOTE: The following form should be completed and taken with you to your physical. It must be submitted to the school office on or before August 16th.

	ATHLETIC ELIGIBILITY: Students' grades will be averaged and checked WEEKLY. If a student fails one subject, he/she may still practice but not play in the game until the average is brought up to passing (70 or above). If a student is failing two or more subjects, he/she may neither practice nor play until the grades are brought up to passing.
